It is prudent to review policies and procedures after an instigating event; however, even in the absence of events, policies and procedures should be reviewed regularly. Looking to evidence, the literature, and professional organizations’ and accrediting bodies’ publications, the facility should strive to craft policies and procedures that are custom-tailored to their goals and aims. A standardized review process should be created that involves stakeholders—those whose responsibility it will be to follow the policy or procedure. Other reviewers should include members of risk or legal counsel, staff members responsible for regulatory compliance and accreditation, and external experts in the field.
